Website Free-Call Form
Our public free-call form requests only basic contact and logistical information to determine service fit:

- Email address
- Municipality or postal code
- Preferred contact time
Visitors should not submit detailed personal health information through this public form. Information submitted through the form is sent to Restore at Home by email after server-side validation. The site does not intentionally store completed lead submissions in its application database, though email systems may retain messages according to standard operational and provider retention practices. Form values are not intentionally sent as analytics or advertising event parameters.
Clinical Information
Once an individual becomes a patient, clinical and health information may be collected and maintained as required for care, documentation, billing, authorized communication, and legal or professional obligations. This information is collected and managed separately from the public marketing website through appropriate clinical and administrative processes.
Personal Health Information
Personal health information is handled through the clinical and administrative processes used to provide care. The public website and free-call form are not a clinical chart and should not be used to send detailed health information.

Cloudflare Turnstile
We use Cloudflare Turnstile on our website to prevent abuse and spam submissions. When you interact with our forms, technical, browser, and network signals, as well as a verification token, may be processed by Turnstile. Turnstile is a third-party service, and its use is subject to third-party privacy terms.
Website Cookies, Analytics, and Advertising
Our website uses standard technologies, including cookies, to improve performance and measure engagement. We use:
- Google Analytics 4 for aggregated website traffic and engagement measurement.
- Google Ads conversion measurement to evaluate the effectiveness of our advertising campaigns.
- Google Search Console for aggregate search and indexing information.
Non-essential Analytics and Advertising tags are consent-controlled. Technical browser, device, page, referrer, and advertising identifiers may be processed when consent is granted. We do not intentionally send form-field values or personal health information as analytics or advertising event parameters.
